How to Create Care Plan Templates



What is a Care Plan Template?

A care plan template serves as a blueprint for benefits, goals, and tasks, helping individuals improve their circumstances and achieve positive outcomes. Care Plan Templates may be created to address health, safety, well-being, and other personal and social issues. For example, an employment assistance care plan template may be created and utilized so that caseworkers and other staff members can accelerate the development of an individualized care plan for an unemployed individual. 

What is included in a Care Plan Template?

  1. Goals: These are the desired outcomes for the individual receiving care. They can be broad, like "Improved Quality of Life for Seniors", or more specific, like "Healthy and Active Aging". 
  2. Benefits: These are the resources, services, or support that will be provided to help the individual achieve their goals. Examples include meal delivery, home care assistance, or access to support groups. 

    Goal Definitions must be established before the Care Plan Template can be completed.

What are Goal Definitions (Goals)?
In Salesforce, a Goal Definition is a template for defining specific objectives within a care plan. These definitions are reusable components that capture common information, such as the goal's name, description, and category. When a care plan template that uses a Goal Definition is applied, the definition is instantiated into a Goal Assignment, copying over the predefined details.  Essentially, the goal definitions become the goals included in the Care Plan Template.

What is included in a Goal Definition?
  1. Name: A descriptive label for the goal (e.g., “Improve Patient Engagement”).
  2. Description: A more detailed explanation of the goal’s purpose and what it aims to achieve.
  3. Category: A classification to help organize and group similar goals (e.g., “Clinical,”             “ Lifestyle,” “Financial”).
  4. Goal Value (Optional): A monetary value assigned to the goal, often used for reporting and tracking progress.
  5. Goal Target (Optional) A specific numerical value or percentage that represents the desired outcome for the goal.

Goal Definitions are foundational to care plan management; they provide a structured way to define and track progress towards desired outcomes for individuals or groups.  They are particularly useful in scenarios where multiple individuals or groups work towards similar goals, allowing for consistency and efficiency in care plan creation.         


User permissions are needed to create a care plan template: Care Plans Access permission set.


Step 1 - Create Goal Definitions
  1. From the App Launcher, find and select Goal Definitions.





  2. Click New.


  3. Enter a name and description.
    a. Note: The description is not directly linked to the care plan, but any information added in the description will be visible in the care plan when it is created.
  4. Select a Type, Top Goal, or Intermediate Goal.

What you need to know about "Top Goals" and "Intermediate Goals" is that they are used to track progress towards desired outcomes within care plans. Top goals represent the ultimate, overarching objectives, while intermediate goals are smaller, actionable steps that contribute to achieving the top goal. 


Top Goals:
  • Represent the final, desired outcome for a participant. 
  • Track the ultimate, high-level results a participant aims to achieve. 
  • Examples include "Secure stable housing," "Obtain employment," or "Improve overall health". 
Intermediate Goals:
  • Break down the top goal into smaller, more manageable steps.
  • Serve as checkpoints or milestones on the path to achieving the top goal.
  • Examples include "Attend three job readiness workshops," "Complete resume and cover letter," or "Find three potential housing options."
Start by defining your main goal, which is the Top Goal. Next, identify all related Intermediate Goals that support the Top Goal. When it comes time to add goals to the Care Plan Template, the Intermediate Goals will automatically be included when you select the Top Goal.  

5. Set the status to Active.
6. Click Save.




















Step 2 - Create the Care Plan Template Record
  1. From the App Launcher, find and select Care Plan Templates.



  2. Click New.
  3. Enter a name for the template that reflects the assistance scenario. 
    a. For example, Emergency Housing or Employment Stability. 
  4. Select Draft as the status.

Note: You can’t edit a care plan template after it is published.  Don’t publish the template until you add goals and benefits.

     5. Save your changes.

Add Care Plan Template Goals
     
    6. From the Care Plan Template, click on the Related tab.
    7. Locate Care Plan Template Goal and click New.
 
   8. Select a goal definition, set the priority.
      a. If using a top goal linked to intermediate goals, search and select that top goal in the Goal Definition            field.
   9. Save your changes.

Note: Search and select the Top goal when adding goals to the template; the intermediate goals will automatically be populated with the Top goal.

     10. Remain on the Related tab on the Care Plan Template and locate Care Plan Template Benefits and                 click New.
Note: The benefits record(s) must be created before adding the benefit to the care plan template.
  
Note: When adding a benefit to the care plan template, if the client does not have an existing program enrollment record for the benefit being provided, the system will automatically create the program enrollment for the applicable program.

      11. Search for and select a benefit, then set its priority.
      12. Save your changes.

Note: You can add as many Benefits as needed to the Care Plan Template
     13. To see the hierarchy of the template’s goal definitions and benefits, go to the Preview tab.
     14. When you’re satisfied with the care plan template, publish it.
         a. From the Details tab, click on a pencil icon to edit the record.
         b. Select Published as the status.
     15. Save your changes.
